Find Out If You're Eligible
Eligibility for services at RotaCare is based upon Federal Poverty Levels. Patients are financially eligible if your annual income is less than 200% of that level. They must be uninsured and ineligible for Medicare, Medicaid or any other type of health care program. Your medical needs must also be assessed. A final determination of financial and medical eligibility can only be made after completion of the intake process.
Intake Requirements
RotaCare serves people who do not have health insurance or the means to pay for their health care. Before new patients are accepted, they meet with a social worker to evaluate their income level, their eligibility under the Affordable Care Act and for public health insurance programs such as Medicaid, Medicare or Family Health Plus.
Specific documentation requirements are discussed with a patient during the intake process. In addition to financial eligibility, each patient is evaluated for medical appropriateness. Recognizing that every patient’s circumstances are unique, we carefully evaluate each patient on a case by case basis to determine eligibility.
RotaCare strives to meet the needs of all eligible patients, but we are an all volunteer operation with limited resources, so we ask that new clients please be patient if we are unable to assist you immediately.
